The practices Learning Disability patients are supported by our Doctors and Nurses, our lead LD nurse is called Sara Thompson and she provides an annual assessment for all patients with Learning Disabilities.
We will do your LD health check usually once a year in the month of your birthday and at this appointment you can ask any questions about your health, the nurse can refer you to a specialist team if it is needed.
A general health check will be done to make sure you are getting the best treatment our nurses can give you easy read leaflets to help you understand the tests that you may need. Your Carer can also get support from our team too. You will leave with a Health Action Plan that you can follow to help with your health.
Please ensure you attend the surgery for your health check when you are invited.

We have recently had a Learning Disability Health Assessment Audit carried out at our surgery to evaluate how well we do with our Health Assessments. You will be pleased to know, we are at the highest level which is GOLD. This means we are supporting patients with learning disabilities to the best of our ability.
